By Thomas Clouser Genetically modified organisms, or GMOs, are any plants or animals that contain genetic structures that have been manipulated by scientists. Using genetic engineering, scientists can pinpoint beneficial traits in any organism, in terms of added nutrition, better flavor, or greater ability to fight pests or diseases, and incorporate them into other organisms. This is done by isolating a particular gene responsible for a trait in one organism, removing it, and then transferring it to another organism, where this same gene replicates itself, creating a stronger and more resilient strain of the same organism. Although researchers have been employing genetic engineering techniques in agricultural crops since the mid-1980s, and media coverage of these developments has increased sharply in the past few years. Most people have little awareness of genetically modified foods and the controversies surrounding them. In a 2001 survey conducted by Penn State agricultural economists, 84 percent of those questioned said they either knew little or nothing about GMO foods, or hadnt heard of them at all. Yet these techniques and products have already had an impact on our food system, from producer to consumer, and will continue to make their mark on the worlds food supply. Statistics indicate that as much as 60% of the foods we purchase at the supermarket have ingredients that come from GMOs. Since much of the genetic engineering thus far as been performed on corn and soybeans, this would then include ingredients such as vegetable oils produced from these crops. No matter how diligent one may be in isolating their household from genetically modified foods, it is most likely that all of our readers have consumed GMO produced foods, in the form of vegetable oils and corn flakes, delayed-ripening tomatoes, and pest-resistant vegetables. GMOs are ingredients in salads, pizza, baked goods, and many corn-based cereals. They will be consumed from grocery purchases, and in many restaurants and food stands. While the future of genetic manipulation is uncertain, the business is booming and likely to continue. Pressing global needs to produce more food for a greater population, and producing it all from less agricultural land, implies the need for increased yields per acre. Crops that are less often destroyed by plant diseases, and animals that produce more lean meat for the amount of feed they consume are just two factors being heavily addressed in genetic engineering for agriculture. There is a vast amount of land yet on the globe not being farmed. Future decades will likely see the clearing and development of such lands to meet global food requirements, but environmental pressures from many limit such development. Genetic engineering has proven an affordable short term solution. So what are the risks? What will be biotechnologys long-term effects on the environment? By altering nature through genetic engineering, are we coming dangerously close to playing God? Many of these questions may never be answered. I mean this is a good time to check the health of your site or setting. As healthcare providers, we are dedicated to curing our patients of their ills and keeping them healthy. My question is, Do you have the same attitude about the place you work? Is your site meeting its highest goals? If not, then it is time for a change. I think that as a group healthcare sites are a lot like our patients. Some patients dont work hard at keeping healthy. In fact, many engage in behavior that is very detrimental to their well-being. You probably can easily image such a person. Maybe they are very overweight. Well, there are many healthcare facilities that are like that. They have many processes in which there are steps which add no value, that are a complete waste of time and effort. Then, there are patients who do a lot of the right things. They eat correctly, get a good amount of exercise, and generally have a positive outlook. There are a few healthcare facilities that are fit too. What does a healthcare site in not so good shape look like? These sites, I believe, have as their goals just to meet their compliance standards, those set by the state, JHACO, or the FDA or some other body. As long as they are being accredited, they are satisfied. Their attitude may be, If it isnt broken, dont change it. They are like patients who are content to just sit on the couch, watch TV, and eat whatever they like as long as they dont feel poorly. What does a healthcare site in good shape look like? I believe that such sites are continually looking for ways to better themselves. They engage in continuous quality improvement activities. They are really concerned about the health and attitudes of their patients and clients. They use surveys and they question and listen. Some even use a tool called quality function deployment when designing new facilities or programs. Besides listening to patients, they pay close attention to their employees. They actively seek ideas from the employees on how things could be done better. They want to have their employees enjoy working at their site and support its mission. The leaders of such sites actively reflect this attitude. Further, such sites seek ways to improve the processes at their site. They cut waste and save time and improve the bottom line. What are the results of these positive and active approaches? Our patients are happier, fitter, and find time to do the things they want. They generally live longer with fewer health problems. Healthcare sites which are actively engaged in continuous quality improvement programs generally find that they have the time to accomplish their objectives and dont feel rushed, they have an improved bottom line, and they rarely experience adverse events. The doctors dont have to work impossible hours to accomplish all they want. They work as a team. Let me encourage you at this time to sit down with your colleagues and leaders and assess the health of your site. The nerves to the stomach and the outlet valve (pylorus) remain intact with the idea of preserving the functions of the stomach while reducing the volume. By comparison, in a Roux-en-Y gastric bypass, the stomach is divided, not removed, and the pylorus is excluded. The Roux-en-Y gastric bypass stomach can be reconnected (reversed) if necessary. Advantages of the Sleeve Gastrectomy Surgery: The stomach is reduced in volume but tends to function normally so most food items can be consumed, albeit in small amounts. Eliminates the portion of the stomach that produces the hormones that stimulates hunger. No dumping syndrome because the pylorus is preserved. Minimizes the chance of an ulcer occurring. By avoiding the intestinal bypass, the chance of intestinal obstruction (blockage), anemia, osteoporosis, protein deficiency and vitamin deficiency are almost eliminated. Very effective as a first stage procedure for high BMI patients (BMI>55 kg/m2). Limited results appear promising as a single stage procedure for low BMI patients (BMI 35-45 kg/m2). Appealing option for people with existing anemia, Crohn’s disease and numerous other conditions that make them too high risk for intestinal bypass procedures. Can be done laparoscopically in patients weighing over 500 pounds. Risks of the Sleeve Gastrectomy Surgery Potential for inadequate weight loss or weight regain. While this is true for all procedures, it is theoretically more possible with procedures that do not have an intestinal bypass. Higher BMI patients will most likely need to have a second stage procedure later to help lose the rest of the weight. If possible take a photograph of the wedding dress along with you, or at the very least have a detailed description of the dress. The florist needs to know where any special detail is on the dress so as not to cover it with flowers. Try and take swatches of attendants fabric with you. Your flowers should complement the outfits, not match. Let the florist know what budget you have in mind so they can work to it. Together with your florist you will work out the design and quantity of wedding flowers you desire. Catching Fish with Books, Not Just Hooks

By Scott McDuffee Can you really catch a fish with a book? Well, sort of… The BEST way to become a better fly angler is to get real honest-to-goodness time with wet feet and a wet line, preferably with someone knowledgeable on that particular piece of water. That being said there is a wealth of literature to be discovered on the topic. From where I sit typing this, I am eyeballing a full shelf of books specific to fishing and fly-fishing. Soft cover and hard, in every shape and size - mine have weathered pages and worn bindings from frequent use. Frankly, being in the “weekend warrior” category, I dont get to fish as much as I would like. Some philosopher fisherman (those two words are almost synonymous, pardon the redundancy) said that the “earth is two-thirds water, so a person should spend two-thirds of their time fishing”. This is perfect logic. However, the execution of said logic cannot possibly come to fruition in my life due to kids school events and sports, work and related money gathering activities, family commitments and responsibilities, house and yard work, and other conspiratory distractions from fishing. My opinion is that whoever came up with the two-thirds theory must have been retired and had a lenient spouse to boot. My point is that books are great for when youre wishin you are fishin, as well as trying to get better connected to the sport you love. There are many book topics beyond even the following “how tos”: how to cast, how to read the water, how to find fish, how a fish sees and eats, how to tie flies, how to build a rod, how to get a drag free drift, how to ad infinitum. As I like to say, it may be that “the early bird gets the worm” but it is the “second mouse that gets the cheese”. Oops, there are those clichs again, now in simpler terms - Why try to re-invent the whole dang thing yourself, when there is plenty of knowledge out there? Steal as many of those nuggets O wisdom as you can. Reading is a great jumpstart to a lifelong love of the artful pursuit. There are also many books that expand into story telling and history sharing; the best of these make you feel like you are hearing stories around a campfire from an old friend. You cant go wrong with blending humor and history. Given the amount of time we fish compared the amount of time we actually “catch”, a fisherman must connect to the humor and philosophy of it all or seek a more “efficient” sport. Also, there are specific geographic guides that enlighten the interested, and dozens of other topics. It will take a good dose of reality and a brave heart to stop denying what really happens. Being overweight can be used as an excuse for being unhappy, especially when you do not do anything to help yourself. If you are concentrating on another activity while you are eating you are more likely to overeat because you are not fully aware of how full you are feeling. This factor can be difficult to change because it is not a conscious action. Try to only eat when you dont have a lot of other distractions. Sit down, eat slowly and enjoy the food you are eating, and remember that it is not always necessary to go back for seconds. It takes 20 minutes for food to reach your stomach and for your brain to register that you are full. Cravings? The famous Pavlovs dogs were conditioned to eat at the sound of a bell, and we human are much the same when it comes to habitual cravings. Like conventional contacts, certain colored contact lenses are also used for astigmatism or bifocals. Colored multifocal (or bifocal) contact lenses are becoming very popular for several reasons. Besides correcting near vision and distant vision, they can totally change the color of eyes or can serve to improve dark or light eye color. Since the lens is used for two distinct purposes, multifocal contacts are designed in various ways. In some styles, near and far corrective parts are seen on the same area of the lens. But in some other designs, the two parts are placed away from each other. There are mainly three common styles - the alternating, simultaneous, and concentric vision styles. In an alternating lens, the distant visual correction is fixed on the top half of the lens, while the near visual correction is on the bottom. This arrangement enables you to look up to distant objects, or look down to read a book. In the simultaneous bifocal lenses, the two corrective parts of the lens are placed very close to the center (almost at the pupil). In a concentric lens, the distant vision part is near the perimeter of the lens, while the near corrective is close to the center. Among the many choices for colored contacts are visibility tints, enhancement tints, opaque color tints, and light-filtering tints. Visibility tint lenses do not affect your eye color. Enhancement tints are used for the purpose of improving the eye’s natural color. Opaque tint lenses are designed to alter eye color, and consist of several tiny colored dots to attain the color change. But, center part is left clear for vision purposes. Light-filtering tints are designed to enhance certain colors and diminish others.
